Why you shouldn’t get a tatoo

Everyone feels and sees the body and immune reaction following a tattoo procedure; the redness and the pain are obvious. When tattoo needles puncture the skin, the immune system treats the procedure simultaneously as an open physical wound and an invasion by foreign matter. Most people think that is about it, but most people who think that are wrong.

The reality is far more complex, and it endures as long as that tattoo ink is there. Once the initial response subsides, dermal cells and a different type of immune cell called macrophages arrive to clear the foreign pigment. But instead, what ends up happening is an ‘Engulfment Failure’ and a permanent negative feedback loop that becomes a permanent source of systemic inflammation:

  • Engulfment Failure: Macrophages recognize ink pigments (such as carbon black or metal salts) as foreign and engulf them via phagocytosis. However, they lack the digestive enzymes to break down inorganic pigment crystals.
  • The “Capture–Release–Recapture” Loop: Unable to degrade the ink, the macrophage holds the pigment locked inside its cytoplasm. When that macrophage eventually dies, the ink particle spills back into the extracellular dermal matrix, only to be immediately engulfed by a neighboring incoming macrophage. This ongoing turnover keeps the tattoo visible in place for decades.

This loop becomes a decades-long source of inflammation. Inflammation, especially a systemic one, is one, if not the main, driver behind a lower quality of life, mental and cognitive impairment, and fatal chronic illnesses and cancers out there.
